Sevita is seeking a compassionate and dedicated Therapeutic Counselor to join our Community & Social Services team within our Healthcare & Medical Services organization. In this role, you will provide person-centered, community-based therapeutic support to individuals and families of diverse ages and abilities, helping them build skills, resilience, and independence so they can live life to the fullest. As a Therapeutic Counselor with Sevita, you will deliver behavioral health and supportive counseling services in home, clinic, and community settings. You will conduct strengths-based assessments, develop individualized treatment and support plans, and provide ongoing therapeutic interventions using evidence-based and trauma-informed approaches. You will offer individual and, when appropriate, family or group counseling, focusing on emotional regulation, coping strategies, social skills, recovery support, and behavior management tailored to each person’s goals. Collaboration is central to this position. You will work closely with an interdisciplinary team that may include clinicians, case managers, nurses, direct support professionals, and external partners such as schools, physicians, and community agencies. You will coordinate care, participate in team meetings, and communicate regularly with families, guardians, and other natural supports to promote continuity and quality of care. You will also connect individuals and families with community resources that address social determinants of health, such as housing, employment, education, and peer support. Key responsibilities include conducting intake and ongoing assessments, creating and updating treatment plans, maintaining accurate and timely clinical documentation in electronic health records, and monitoring progress toward clearly defined goals. You will provide crisis intervention and risk assessment when needed, including safety planning and coordination with emergency or higher levels of care. Throughout your work, you will uphold professional ethics, maintain appropriate boundaries, and ensure compliance with all regulatory, agency, and payer requirements.
