As a Therapeutic Counselor with Sevita, you will play a key role in delivering person-centered behavioral health services that help individuals of all ages and abilities live life to the fullest. Working within the Healthcare & Medical Services department, you will provide therapeutic support in home, community, and clinic-based settings, partnering closely with individuals, families, and interdisciplinary teams to promote emotional wellness, independence, and meaningful life goals. In this role, you will conduct strengths-based assessments, develop and implement individualized treatment and support plans, and provide evidence-informed counseling interventions tailored to each person’s needs. You will facilitate individual and, when appropriate, family or group sessions to build coping skills, improve communication, and support behavior change. Using a trauma-informed, culturally responsive approach, you will help individuals manage mental health and behavioral challenges, navigate life transitions, and access needed community resources. Your responsibilities will include monitoring progress toward goals, updating treatment plans in collaboration with the individual and their support network, and maintaining accurate, timely clinical documentation in accordance with regulatory, ethical, and organizational standards. You will communicate regularly with case managers, physicians, educators, and other care providers to coordinate services, advocate for the individual’s needs, and ensure continuity and quality of care across settings. You will also provide psychoeducation and guidance to families and caregivers, helping them understand behavioral health conditions and develop strategies to support their loved one’s progress at home and in the community.
